We are fully licensed by the state of Florida and we are Medicare certified.

Some services at Middleburg Surgery Center may be completed by other health care providers who may use different billing processes, health insurances, and health maintenance organizations (HMOs).

Out-of-Network Providers
Patients whose insurance is out of network from Middleburg Surgery Center may still be able to get an adjustment on their liability. This is only possible when Middleburg Surgery Center is not actively prohibited from offering out-of-network adjustment under state law, Federal law, or the insurance provider. This adjustment will vary based on the insurance plan. Delinquent accounts may lose the adjustment.
